OSLO, Norway – An investigation of a fatal accident onboard the jackup Maersk Interceptor last month has identified inadequacies in systems and processes for materials handling.

The Petroleum Safety Authority (PSA) says the incident occurred during the installation of a new seawater pump, while the rig was at work for Aker BP on theTambar field in the North Sea.

One person fell into the sea and was killed, and another suffered serious injuries.

A team from the PSA posthumously conducted investigations on the rig. Based on findings to date, Maersk Drilling Norge has been set the following tasks:

1. Review systems for follow-up and use of portable lifting equipment on all facilities in use on the Norwegian continental shelf, including internal control, training of personnel using such equipment, and transfer of experience with such equipment.

2. Systematically identify and review all operations related to the materials handling process for installation of seawater pumps on all facilities in use on the Norwegian continental shelf; identify risks and implement necessary measures.

The deadline for compliance with the order is Feb. 28.
